AIDS Community Care Montreal (ACCM) is seeking a Treatment Coordinator to join our team! ACCM is the only English-speaking community organisation that offers prevention and support services to people living with or affected by HIV and/or hepatitis C in Quebec. We work in collaboration with our many communities to build a compassionate and caring response to HIV and hepatitis C and facilitate education that equips people with the knowledge and skills needed to make informed decisions around their sexual health.

The main objective of the Treatment Coordinator is to facilitate healthcare navigation and access for people living with HIV/Hep C, following GIPA/MEPA principles. The Treatment Coordinator’s main responsibilities are:

• Act as a resource for our HIV/hep C positive membership to acquire and utilise HIV treatment information, to navigate healthcare (i.e., access to care including RAMQ, physician, and treatment), to navigate through disability benefits / social solidarity / social assistance administrative process, and to access other health resources.
• Facilitate the HIV and ageing workshop series through a by and for model with Members, including training, capacity building, and support for facilitators.
• In collaboration with the Volunteer Coordinator and the One-on-One Support services coordinator, run and evaluate the Buddy program, including training volunteers, making pairings between volunteers and Members, as well as providing support and follow up for both volunteers and Members involved in the program.
• Support the Support Services Department providing intakes, holistic needs assessments and follow-up support and referrals to our HIV/hep C positive membership, including psychosocial, mental health, access to healthcare, food security, housing, employment, etc.
• Collaborate with other programming staff on joint projects and activities.
• Participate in internal and external committees as needed.
• Performs all other related tasks related to the nature of the position.

Our ideal candidate has all or a combination of the skills and experience below:

• Degree in social work or community development.
• Knowledgeable about the Quebec healthcare system.
• Demonstrated experience in (peer) counselling and individual intervention, support and follow up.
• Understands the value of peer support and peer engagement, particularly in the context of GIPA/MEPA.
• Strong understanding of self-determination, well-being and empowerment of ACCM’s membership.
• Empathetic and a good listener, non judgemental and open to diverse histories, lifestyles and backgrounds of our Members.
• Conflict management skills and clear boundary settings.
• Demonstrated experience in planning and facilitating workshops.
• Strong data entry skills and database management utilising a client-member database program (Penelope).
• Ability to maintain accurate records of support care to assist the Support Services Team in providing individualised care for our members, in addition to assisting in reconciliation of information for government and grant reporting.
• Ability to work autonomously and as part of a team.
• Is multilingual, at minimum, perfectly fluent in English, both written and spoken, and conversational French, written and spoken.
• Experience working with communities who are disproportionately affected by HIV, hep C, and other STBBIs such as queer, trans, migrant, Black, Indigenous, Latinx, and Arab communities, as well as people who use drugs.
• Has a fervent belief in harm reduction, and a proven history of practising harm reduction and anti-oppression-based interventions.
• Is knowledgeable about sexual health, HIV, hepatitis C, STBBIs, GIPA/MEPA approaches, and the social drivers of health.
• Believes in the organisation’s mission and vision, and will act as an ambassador for ACCM to our partners and external committees.

Specifications
ACCM operates from Monday to Thursday, 10:00 am-6:00 pm, however this position does require working occasional evenings and weekends during group activities or events. The starting pay rate for this position is $21.37 per hour, 32h per week, and includes 3 weeks of paid vacation (on top of 2 weeks over the end of year break), 10 sick days per year, as well as a group health insurance plan (available after the 3-month probation period).
